Assesment of Medical Fitness
This form has to be filled out by a doctor in the Czech Republic. The form is valid for two years. You can use the form linked below, or ask your doctor for a Posudek o zdravotní způsobilosti dítěte. Please send it to the Admissions office or scan and send it by email to [email protected].
Enrollment Contract
The Enrollment Contract is signed during the New Family Orientation.
Legal Guardianship Contract
If a CISP student lives with a legal guardian, the Legal Guardianship Contract and Legal Guardianship Appointment form should be filled out. The Legal Guardianship Appointment form needs to be signed and notarized.
Withdrawal
According to policy, parents must give CISP two months notice for early withdrawal of a student to avoid financial penalty. If you are withdrawing your student, please complete this online form:
Families who plan to temporarily leave CISP, for example to spend a semester in their passport country, should inform CISP using the following online form:

Scholarship Application
Scholarship deadline is 30 June. The following students are eligible to apply for scholarships:
- Current students who have completed all steps of re-enrollment.
- New students who have received an offer of enrollment.
